In-Prison Pre-Release Case Management

Returning home begins with preparation. Our in-prison case management helps participants identify needs and plan for their transition into the community.

This work includes our PAVE Application process and preparation for support after release, with a focus on continuity of care and a more stable return home.

Through Partnership HealthPlan of California and Kaiser Permanente, we provide enhanced care management, housing navigation, and housing sustainability services for eligible members.

Housing Navigation

Finding safe and stable housing can be genuinely challenging for many. We help participants explore housing options and navigate the steps toward securing a place to live.

Stability takes ongoing support. We help participants address challenges that may affect their housing and connect with resources that support long-term housing stability.
