Returning to the community brings many decisions. Having a place to stay can provide a foundation for focusing on the next ones.

Through Hazel’s Tranquility Place, we offer a six-month housing program that supports the process of rebuilding stability and preparing for the next chapter.

Space to focus on what comes next

A housing program can offer a starting point during a period of change. Participants may be thinking about employment, care, personal responsibilities, and future housing.

Each person’s priorities will look different. The shared goal is to move toward greater stability.

Understanding the different kinds of housing support

Match2Hire’s services address housing needs in several ways:

  • Six-month housing through Hazel’s Tranquility Place provides a temporary housing foundation.
  • Housing navigation helps eligible participants explore options and navigate steps toward securing housing.
  • Housing sustainability helps eligible participants address challenges that may affect their ability to maintain housing.

Housing navigation and sustainability services are available for eligible members through Partnership HealthPlan of California and Kaiser Permanente.
