Connecting preparation with ongoing support

Our pre-release work includes the PAVE Application process and preparation for services after release. The focus is on continuity: helping participants think through what they will need when they return home and how community support may fit into that plan.

A plan does not remove every challenge, but it can provide a clearer starting point.

The transition continues after someone comes home. Match2Hire’s services include employment programs, digital literacy training, enhanced care management, and housing support.
